Forms & Policies

All Graduate School forms can be found below.  Forms related to the admissions process (applications, residency, etc.) can be found under the Admissions Forms tab.  Student related forms (petitions, transfers, etc.) can be found under the Academic Forms tab. 

Where do we send the Forms?

Send by Email: Completed admissions forms should be emailed to grad.school@unh.edu. Completed academic or financial forms should be emailed to grad.studentrecords@unh.edu. Be sure that the file name begins with your last name.

Send by Mail: Mail completed forms to UNH Graduate School - Thompson Hall, 105 Main Street, Durham, NH 03824.
